As part of a typical assessment with a new patient it is important to ask for a current medication list as well as any supplements they might be using.  When we encounter a patient who is taking herbal supplements it is important to ask why they are taking them and what impact it is having on their overall health status.  There is an assumption among consumer that given supplements are “all natural” or derived from plants that they must be safe.  Unfortunately, this big misconception can lead to complications when supplements are combined with prescription medications and drug interactions can cause adverse effects.  Taking an herb or supplement could change the way a prescription medicine works in the body, causing symptoms like an overdose.  On the flip side, taking herbal supplements might cause the medicine not to work at all.  Common medications that have been known to have adverse interactions with herbal supplements are cardiac medications such as digoxin.  Insulin used for the treatment of diabetes and Warfarin used to treat clotting disorders has also been known to have some toxic interactions with herbal supplements (Soloway, 2024).   Another time is it important to ask patients about the use of supplements is when new medication is being prescribed.  If the new medication has a high-risk potential to interact with herbal supplements, then risks vs. benefits must be discussed with the patient.  There are nearly 1500 documented interactions between drugs, herbal medicines, and dietary supplements.  This is a lot to keep track of.  The most important point it to communicate with patients in your care and making them feel comfortable to share all aspects of treatment.  Creating a therapeutic environment based on trust and understanding will most likely be the key to avoiding harmful drug/supplement interactions.
